If you live in our practice area and would like to register, either call at the surgery with your medical card or if you do not have one you can complete Form GMS1 which is available from reception.
You will also need to bring proof of identity for each person wishing to register with the practice, such as a passport, driving licence, you will also need to bring proof of residence, such as a council tax letter, telephone or water rate bill.
